Title : Lalana newborn resuscitation and new perinatal strategy

Introduction: ‘Lalana Newborn Resuscitation’ (LNR) discovered for the first time in the world is the only safest and best non-invasive resuscitation technique in newly borns transiting from fetal fluid filled lungs to well aerated neonatal lungs by continuous distending pressure with sustained nasal oxygen inflation results in quick onset of rhythmic respiration. Definite inherent ethnic variation exists between Asians and Caucasians populations

Aims & Objective: Thee most critical task in newly born resuscitation is lung aeration by clearing lung fluid for gaseous exchange. Oxygen is the only specific therapy for hypoxia to decreases central apnea and promote regular breathing pattern by sustained nasal oxygen inflation as opposed to IPPV by NRP. Also Ethnic Asian guidelines are ideal for improving outcome.

Material & Methods: Grading of newborns I – V based on Pulse oximetry score from zero to +5, respiratory and heart rate. Implementation of Protocol I and Protocol II resuscitation in term and preterm <32 weeks gestation <1250g newborns with continuous distending pressure by sustained nasal oxygen inflation shifts fetal lung fluid causing uniform alveolar recruitment with large reduction in Pulmonary Vascular resistance (PVR) facilitating reflex vital cardio-vascular changes adapting to extra-uterine life has both scientific and physiological basis.

Results: The incidence of birth asphyxia was 21.4% among 1,383 singleton live births with 178 asphyxiated new-borns in 830 deliveries attended. LNR method resulted in establishing rhythmic respiration within 1-3 minutes, of which 97.7% occurred within 1 minute and remaining 2.2%, within 2-3 minutes, with continuous monitoring by pulse oximetry till SpO₂ >96%, rhythmic respiration, heart rate >100 beats per minute (bpm), all had uneventful neonatal period. Asian Due Date at 38 weeks gestation will improve outlook for both baby and mother.

Conclusion: Effective resuscitation by Lalana Newborn Resuscitation (LNR) is based on scientific principle of application of Continuous Positive Pressure Ventilation (CPPV) by sustained nasal oxygen inflation with real time SpO₂ and heart rate (bpm) monitored by pulse oximetry to grade hypoxic/asphyxiated newborns to determine oxygen flow rate saving lives so children should be normal and thereby reducing perinatal, neonatal, Infant and consequently under-five child mortality and morbidity rates. Introduction of Asian Due Date into clinical practice will prove beneficial to both baby and mother.
